    • What If I received another vaccine?
      If you received any other vaccine within the last 14 days, you will not be eligible to schedule an appointment at this time. 
    • Where Can I submit my vaccine records?
      Notify our Compliance team when you have received the first and second vaccines by emailing hrfax@truecareny.com or by faxing 347-573-9062 your proof of vaccine index card.
    • What do I need to bring with me to my appointment?
      Make sure to bring an acceptable form of identification such as a Driver’s License, Passport, or Social Security Card on the day of your appointment.
    • Whom can I contact with any questions or concerns? 

      If you have any questions about the vaccine, please call our COVID-19 Hotline at 929-474-9322 and a True Care Nurse will be in touch.
