Home Care  Scheduling


Job Description

The Home Care Scheduling Coordinator works together with the Coordination team and is responsible for coordinating caregiver placement and visit confirmations. He/she functions under the direction of the Supervisor.

What You'll Do:

Coordinates placement of services between clients and the paraprofessional staff.

Maintains and documents communication with paraprofessionals.

Maintains a schedule of all services per month.

Verifies billed services against requested services; Helps maintain patient files.

Assists with screening of caregivers.

Maintains patient confidentiality and privacy with dignity and respect.

Participates in patient case conferences, in-service programs and orientation.

Communicates with the Coordinator weekly regarding scheduling, availability, attendance, and assignment.

Answers telephone, records and relays messages to appropriate personnel.

Manages telephone calls in a supportive, reassuring manner for patients, families, physicians and others.


Must have HHA eXchange software experience

Must have Home Care Scheduling Experience
