Job Description

The Home Care Scheduling Coordinator works together with the Coordination team and is responsible for coordinating caregiver placement and visit confirmations. He/she functions under the direction of the Supervisor.

What You'll Do:

  • Coordinates placement of services between clients and the paraprofessional staff.
  • Maintains and documents communication with paraprofessionals.
  • Maintains a schedule of all services per month.
  • Verifies billed services against requested services; Helps maintain patient files.
  • Assists with screening of caregivers.
  • Maintains patient confidentiality and privacy with dignity and respect.
  • Participates in patient case conferences, in-service programs and orientation.
  • Communicates with the Coordinator weekly regarding scheduling, availability, attendance, and assignment.
  • Answers telephone, records and relays messages to appropriate personnel.
  • Manages telephone calls in a supportive, reassuring manner for patients, families, physicians and others.


  • Must have HHA eXchange software experience
  • Must have Home Care Scheduling Experience
