Home Care Scheduling Coordinator

Job Description

The Home Care Scheduling Coordinator works together with the Coordination team and is responsible for coordinating caregiver placement and visit confirmations. He/she functions under the direction of the Supervisor.

What You'll Do:

Coordinates placement of services between clients and the Caregivers.

Maintains and documents communication with Caregivers.

Maintains a schedule of all services per month.

Coordinates placement of Caregivers based on patient request.

Maintains patient confidentiality and privacy with dignity and respect.

Answers telephone, records and relays messages to appropriate personnel.

Manages telephone calls in a supportive, reassuring manner for patients, families, physicians and others.

Be the liaison between the patient, patient's family, contract & agency.

Manages the care service provided for their census.

Manages all aspects of attendance by confirming caregiver attendance, monitoring missed visits and missed attendance.

Being the advocate for all patients.

Communicates with the team lead about concerns, accusations or difficult to staff cases.


Bilingual Uzbek speaking required.

Outgoing, positive personality.

Professional, good phone and communication skills.

High School Graduate, prior experience with HHA eXchange a plus.

Excellent communication and interpersonal skills.

Being a team player by participating in on-call rotation with other team members.

Accuracy and attention to detail.

Ability to prioritize and manage time to help reduce overtime.
